Our 6 Step Process to Collaborate with FEI
STEP ONE
Review our website and schedule an introductory meeting to learn how our program services can improve patient health outcomes.
STEP TWO
Secure funding and sign BAA and Service Agreement documents.
STEP THREE
Share awareness of program with staff and educate how to screen and refer patients for food as medicine.
STEP FOUR
Send FEI referrals and visit with program participants at least every 6 months
STEP FIVE
Two-way data sharing to determine the effects and outcomes of the program services.
STEP SIX
FEI shares de-identified reports utilizing data to assist in securing additional funding and implementing any program changes.
Food is Medicine Access
Home Program
Food Is Medicine Access Home (FIMH) program is home delivery of nourishing food as medicine for patients experiencing food insecurity and diagnosed with prediabetes, Type 2 Diabetes, top 9 food allergy, and/or celiac disease.

Produce deliveries can be once a month, twice a month, or four times a month. -
DATA + EDUCATION
Regardless of the program selected, data reports are provided. All patients have access to health app for culinary education.
